Vertical banded gastroplasty (VBG) is one of the recognised methods of the surgical treatment of obesity, although it is
associated with some complications which require further reoperations, as every bariatric procedure. Among them,
intolerance of restriction is one of the most difficult problems in revisional surgery. Usually VBG could be converted into
malabsorptive procedure in such cases. Band removal could be offered to patients who do not accept the above conversion.
It leads to spontaneous dilatation of gastric stoma and disappearance of restriction, but it is also the cause of uncontrolled weight gain. We present a patient with intolerance of restriction after VBG who underwent partial internal transection of the gastric wall in the other department. The above procedure caused band migration and further surgical consequences.
