The diagnostic difficulties are presented in the case of dexter uterine adnexa tumor proceeding with increased levels of Ca-125 marker in a 25-year-old woman. She had a 6 month history of low-grade fever and non-productive dry cough associated with gradually increasing abdominal pain. Ultrasonography showed the right adnexal focal change. She was qualifed for exploratory laparoscopy but during a routine, preoperative X-ray examination of the chest pulmonary intestitial changes were found suggesting a specific process and becouse of that we set suspected pulmonary tuberculosis. The patient was placed on three-drug antituberculous therapy and had a complete recovery.
