Heparin therapy in patients with renal impairment

This is a review of recent data concerning unfractionated and low molecular weight heparins, their mechanism of action and dosing in patients with renal insufficiency. Differences between heparins are provided in their mechanisms of anticoagulant action and side effects. Practical guide to monitoring of heparins particularly in patients with renal impairment is provided. New data on pentasacharides and their comparison with low molecular weight heparins are also presented.
