Otogenic brain abscess are the most serious complication of purulent chronic otitis media. From 1953 to 2009 101 patients were treated in the ENT Department of Poznań University of Medical Sciences. The paper describe the evolution of brain abscess management during last 56 years. All patients were treated surgically and the brain abscess was drained according to Zakrzewski method. Introduction of neuronavigation in this procedure is very helpfull and aid in minimal invasive approach to the brain abscess. A case of a 22 years old man presenting a brain abscess in the temporal lobe is described.
