Introduction:
Xanthogranuloma necrobioticum is a rare, chronic granulomatous disease manifested by the presence of papules and yellowish nodules, most often located in the orbital area. Because of its rarity, the diagnosis and treatment of this dermatosis may be difficult, therefore, the authors found presentation of a case purposeful.

Case report:
A case of a 57-year-old male with xanthogranuloma necrobioticum coexisting with monoclonal gammopathy of undetermined significance and neutropenia of undetermined etiology is presented.

Conclusions:
Patients diagnosed with xanthogranuloma necrobioticum should be under long-term observation considering the possibility of co-occurrence of a hematological disease. Achieving therapeutic efficacy may be difficult and requires a complex therapeutic strategy.
