Chronic invasive bacterial sinusitis is a rare but serious condition that can lead to life-threatening complications, such as a brain abscess. We report a unique case of a 59-year-old patient with no prior history of otolaryngological treatment who presented to the Emergency Department after experiencing epileptic seizures. Subsequent imaging studies revealed chronic invasive bacterial pansinusitis complicated by a brain abscess. The patient underwent functional endoscopic sinus surgery (FESS) followed by neurosurgical evacuation of the abscess. Histopathological and microbiological evaluations revealed chronic sinusitis with Streptococcus intermedius infection. The patient fully recovered with no residual neurological deficits. This case emphasizes the importance of early diagnosis and management of chronic sinonasal disease to prevent severe intracranial complications.
