title="Chronic intoxication in family doctors\’ patients \– rules of diagnosing and documenting",
abstract="There are several sources of toxic substances in modern societies: abused substances, chemicals used in industry, agriculture and food additives, medications, and sometimes also poisons administered with criminal intention. Diagnosing in cases of chronic intoxications is more difficult than acute ones, because in most instances the symptoms and signs are highly non-specific, their occurrence and increasing is staggered and difficult to notice. That is because the key role in early diagnosing chronic intoxications cases plays family doctors\’ medical documentation. It consists of information accumulated for long periods by a physician who knows patients personally and it summarizes data on diagnostics and treatment obtained from various sources. Information contained in these medical documents may have great non-medical importance, especially as proofs in court cases.",
