The most common proposed aetiologies are wound implantations caused by surgical technique and instrumentation, „chimney effect” and the impact of pneumoperitoneum on local immune reactions. The incidence of port-site metastases is actually comparable to the incidence after conventional open surgery. We describe techniques for preventing port-site implants, for example: fixing trocars to the abdominal wall, avoiding of touching the tumour, high vascular ligation, intraoperative colonoscopy and intraluminal irrigation with 5% iodine povidone, specimen isolation before extraction from the abdominal cavity, and intraperitoneal and trocar-site irrigation with a tumoricidal solution.
