Ambulatory blood pressure monitoring – usefulness in routine medical practice

In recent years ambulatory blood pressure monitoring (ABPM) has found a wide application in routine clinical practice. Despite some limitations ABPM demonstrates significant advantages over clinic blood pressure measurements. First of all, greater reproducibility and a possibility of circadian blood pressure rhythm assessment should be listed. It was demonstrated that ABPM measurement correlates more closely with end-organ damage than clinic readings do. It is also proved that increased blood pressure variability and lack of nocturnal blood pressure decline are independently associated with poor cardiovascular prognosis. While ABPM has been widely applied in diagnosis and treatment of arterial hypertension some particular indications have been established. These are: white-coat hypertension, resistant hypertension and suspected lack of nocturnal blood pressure decline.
