Visits of chronically ill patients are making 80% for counsel as part of primary care. About 15% out of the ones ill has three or more chronic diseases and 30% of the hospitalization is a consistency of sharpening the clinical state in this group of patients.
Systems of financing the chronically ill patients care were presented in the paper and potential economic benefits resulting from introducing the system of the chronically ill patients care.
