Differential diagnosis of obese and Cushing’s syndrome

Cushing’s syndrome (CS) is a consequence of a chronic excess of corticosteroids – most frequently of iatrogenic origin. Most cases of endogenous CS are connected with corticotropin producing adenomas. Functional hypercortisolaemia sometimes accompanied by clinical features of CS is called pseudo-Cushing’s syndrome. The most frequent cause of pseudo-Cushing’s syndrome is obesity. Symptoms suggesting CS may be a consequence of metabolic syndrome. A consequence of CS symptoms and metabolic syndrome is difficulties in clinical and laboratory diagnostics. Taking into account that confirmed diagnosis of CS is connected with the possibility of surgery and removal of the primary cause of symptoms, it is crucial to distinguish properly CS and pseudo-Cushing’s syndrome connected with obesity.
