Fever in children under 3 years old is common. In 20% cases of fever there are no clear source of infection in first examination. The serious bacterial infection (SBI) will occur in 1–3% of these children. The risk of SBI is lower in countries where vaccination for Streptococcus pneumoniae and Haemophilus influenzae has been introduced. In this paper we present the criteria for evaluation children with fever >39°C and the algorithms of management.
