There are significant differences in pediatric population in terms of epidemiology of thrombotic events, physiology of coagulation system, pharmacokinetics and pharmacodynamics of heparins and incidence of treatment side-effects, compared to adult patients. These dissimilarities are predominantly expressed in the youngest children and undergo dynamic changes with age. The dosing of heparins is also different in children, compared to adults. Due to objective difficulties with enrolling and maintaining appropriate number of pediatric patients presenting similar clinical profile in randomized trials, many of published recommendations are based on reports of minor reliability. This makes difficult the unbiased evaluation of efficacy of treatment, which often is extrapolated from clinical experience in adult patients.
