Endovenous laser ablation of the popliteal fossa vein (Thiery vein)

Aim of the study: We have used endovenous laser ablation (EVLA) for the incompetent popliteal fossa vein (PFV) since 2007. In this study we analysed 40 cases that we performed from 2011 to 2013, the PFV anatomy, and EVLA features with Slim fibre.

Material and methods: From 2011 to 2013 we revealed PFV insufficiency in 40 patients (40 legs). EVLA (Laser 1470 nm Ceralas 15E and Slim Radial fibres) in all cases combined with Varady’s mini phlebectomy of the varicose tributaries. The follow-up examinations had been accomplished on the first day, after 1 week, and 2 and 6 months. We assessed the duplex ultrasound (DUS) data (occlusion rate) and subjective symptoms such as the presence of neurological lesions and other side effects.

Results: The DUS data allowed us to identify three different types of PFV anatomy. In 45% of cases we detected a straight course of the vein under the fascia. In 35% of cases the vein had a complex structure resembling a tuber. And the third type was detected in 20% of cases and characterised by loop-form veins. A primary PFV occlusion was achieved in 97.5% of cases. In the further follow-up period, in 97.5% of cases ablation and obliteration of the PFV had been achieved, and in only one case a recanalisation of the vein with diameter regression had been discovered.

Conclusions: EVLA of PFV is an effective and safe treatment of PFV insufficiency. A high success rate and a lack of neurological disorders make EVLA of the PFV a good alternative to traditional surgery.
