Diabetic nephropathy affects 30-40% of patients with diabetes mellitus, and after 25-30 years of the disease chronic renal failure that requires dialysis treatment may develop. Complications that appear in dialyzed diabetic patients are a consequence of advanced micro- and macroangiopathy and chronic uraemia. The mentioned complications constitute a large therapeutic problem, because the mortality of dialyzed diabetic patients exceeds 20% a year. In patients with type 1 diabetes mellitus there is a chance to restore physiological control of the blood serum glucose level by pancreas transplantation. The aim of the paper is to present indications and contraindications to pancreas transplantation, principles of qualification of patients for transplantation and donor selection, the technique of the operation, the most common complications, results as well as the state of pancreas transplantation in Poland.
