We describe two middle-aged brothers, ethnic Poles, with pemphigus vulgaris. Despite the frequent occurrence of T and B cell-mediated autoimmunity to desmogleins, familial pemphigus vulgaris seems to be rare. The detection of pemphigus IgG4 deposits in hair follicles in one brother is yet another suggestion that pemphigus pathology affects not only mucous membranes and skin but also skin appendages.
