The article discusses a patient diagnosed with prostate cancer with suspected bone metastases, who suffered from severe background pain (Numerical Rating Scale – NRS 7) despite using concurrently 3 opioids and breakthrough pain: spontaneous and voluntary incident pain intensifying on movement. Effective analgesia was achieved after modifying the background pain treatment and using fentanyl administered by the intranasal route in the therapy of breakthrough pain.
