Osteopenia of prematurity is a common event in preterm infants. It affects 50-60% of infants with birth weight less than 1000 g. The main causes of osteopenia of prematurity are insufficient supply of calcium, phosphorus and proteins, and low physical activity. Clinical manifestation usually appears between the 6th and 12th postnatal week. Decreased linear growth with continued head circumference growth is present. Bone fractures are detected in severe stages of the disease. Diagnosis is supported by biochemical parameters of calcium–phosphorus homeostasis and quantitative bone images such as standard X-ray films, densitometry (DEXA) and quantitative ultrasound bone examination. The most important factor in osteopenia of prematurity prophylaxis is sufficient supply of minerals, protein and energy. Physical activity could also be beneficial.
