Abstract
The Impact of 1550 nm (Er:glass) Fractional Laser Therapy on Facial Skin Quality Improvement in Women with Signs of Photoaging: A Clinical Study

Introduction
Photoaging is a chronic, degenerative skin process induced by prolonged exposure to ultraviolet radiation, leading to wrinkles, loss of elasticity, dull complexion, and enlarged pores.
Objective
The aim of the study was to evaluate the efficacy of non-ablative fractional 1550 nm (Er:glass) laser therapy in improving facial skin quality in women with Glogau grade II–III photoaging.
Material and methods
This prospective, single-arm observational study included 39 women aged 45–55 years who underwent a series of four laser treatments performed at 4–5-week intervals. Treatment outcomes were assessed at four time points: baseline, 8 weeks, 12 months, and 24 months after completion of therapy. The assessment included physician-rated clinical evaluation using a 0–4 scale, patient-reported assessment using a 1–5 scale, photographic documentation, and skin elasticity measurements with a Cutometer in a subgroup of 20 patients. Statistical analysis was performed using the Wilcoxon signed-rank test for paired samples and the χ² test, with p < 0.05 considered statistically significant.
Results
Clinical improvement was observed in 88% of patients, particularly in skin texture, firmness, and complexion. The treatment was well tolerated. Beneficial effects persisted for up to 24 months after completion of therapy. Objective Cutometer measurements demonstrated further improvement in selected skin elasticity parameters during long-term follow-up.
Conclusions
Non-ablative fractional 1550 nm (Er:glass) laser therapy appears to be a safe and well-tolerated method that may improve facial skin quality in women with signs of photoaging. Further controlled studies are needed to confirm these findings.
