Implantable cardioverter-defibrillator, cardiac resynchronization therapy without and with defibrillation function – for elderly?

Clinical trials evaluating of efficacy and safety of implantable cardioverter-defibrillator (ICD) and cardiac resynchronization therapy (CRT) have led to expansion of indications. However, few data are available regarding their use in the elderly patients. The proper selection of patients in older group remains controversial. An individual approach is recommended. No specific age restriction exists but decisions must be based on clinical judgment, and the preference of patients and family. Those elderly patients with multiple comorbidities and limited life expectancy may not be appropriate candidates for ICD or CRT implantation, even if they meet guidelines indications. Older patients are less likely to receive CRT in combination with an ICD (CRT-D).
