Primary vaginal melanoma is a very rare disease. In the literature, there is no consensus on how the best to treat it. A 65-year-old woman case with melanoma of the vagina was described. The primary tumor was located in the third upper part of vagina. Primary lesion was removed. Further treatment was associated with following relapses. In spite of treatment, dissemination of the neoplasm occurred, which was manifested by the appearance of metastases to the lungs, abdomen and pelvis. Currently, the patient is treated by palliative care. The presented case shows rapid and aggressive course of melanoma of the vagina, which, despite early diagnosis and aggressive treatment, has a poor prognosis in this location. Due to the very rare occurence, there is a need to describe each case of melanoma in the vagina to elaborate the most optimal way of treatment and follow-up of these patients.
