Analysis of the statement of incapacity for independent existence in patients with lung cancer considered by the Social Insurance Institution in Wrocław in 2016 in the context of palliative care coverage

Aim of the study
In order to obtain the right to care allowance, a person who is under 75 years old applies to the Social Insurance Institution (ZUS) for a decision of incapacity for independent existence. A large group that applies for this benefit are persons with lung cancer, including those under palliative care. The aim of the work is to assess the functional status and analysis of ZUS decisions about persons with lung cancer and those under palliative care who apply for incapacity for independent existence, in relation to patients who are not under such care, and who also submitted that application.

Material and methods
The material for analysis comprised final decisions issued by the medical examiner or the medical board in 2016 in ZUS in cases of incapacity for independent existence for people with lung cancer.

Results
In 2016, from all persons who applied for care allowance due to lung cancer in Wrocław, 17% of them remained under palliative care. Out of this group, up to 96% of people were given care allowance. At the same time, among people who were not under palliative care (111), the decision about incapacity for independent existence was considered positive in 79 cases.

Conclusions
Applicants under palliative care with lung cancer are referred for palliative treatment too late, while a large group which requires such care is not directed to this form of interdisciplinary treatment at all. The functional status of the body of people with lung cancer can break down quickly, unpredictably.
