Comparison of operative methods performed in prophylaxis of female reproductive organs descensus

Objective: Descensus of female reproductive tract is one of the main reasons of discomfort and pain among women after such gynaecological operations like total abdominal hysterectomy and supracervical hysterectomy which are the most common gynecologic operations performed in the world The aim of our study was the analysis of prophylactic methods performed during subtotal and total hysterectomies to decrease potential risk of descensus of these parts of female genital tract which are left after the operation
Methods: We analysed 189 descriptions of total and supracervical hysterectomies performed in the 2nd Department of Gynaecology and Obstetrics, Medical University of Wrocław from 1999 to 2002 We include 77 patients who underwent supracervical hysterectomies and 112 who were treated with total abdominal hysterectomies We had phone contact with each of these patients and three questions were asked to each of them Questions deals with urine continence before and after operation, pelvis pain or discomfort and the descensus of vagina vault or cervix stump, confirmed by gynecological examination performed by their gynecologists
Results: After performing statistical analysis we stayed that there were no statistical significant differences in urinary continence and pelvis pain in observed group of patients We found that there is a difference in descensus of cervix stump left after supracervical hysterectomy without the bilateral suspension of the cervix on the round ligament of uterus, which was not observed among the patient who underwent total hysterectomy
Conclusions: Supracervical hysterectomy without cervix bilateral suspension on the round ligament may lead to higher risk of cervix stump descensus or prolapse Key words: supracervical hysterectomy, total abdominal hysterectomy, urine incontinence
