Development of adenocarcinoma at the base on recurrent multiforme adenoma pleomorphicum of the parotid gland in 65-year-old man

Adenoma polymorphum (adenoma pleomorphum, tumor mixtus) is the most common benign tumor of the parotid gland. It is often accidentally recognized due to the slow growth and lack of pain. The main treatment option for this type of neoplasm lesions is surgery, usually superficial parotidectomy. The most common complication of the surgery is facial nerve damage, but it can also lead to the formation of a fistula in the area or to the Frey syndrom. Pleomorphic adenoma, after many years of development and growth can transform into cancer. The features indicating malignant transformation are facial palsy, pulling of the skin or its ulceration and cervical lymphadenopathy. Surgical treatment of malignant lesions is total parotidectomy, with or without removal of the lymph nodes of the neck. Complementary treatment is radiotherapy, preferably fast neutrons. This study presents a 65-year-old man with recurrent adenoma polymorphum of the parotid gland. After 9 years a cancer developed on the surface of the primary tumor. Patient didn’t complain of any pain or symptoms indicating malignant transformation of the primary lesion, except for perceptible changes in the form of a hard, shifting nodule. Over the next 14 years he had many recidivations that had been benign as well as malignant character. The occurence of a peripheral facial nerve injury is the effect of numerous operations in the left gland area. The paper presents the clinical data, surgery, scans and histopathology collected during the 23 years of disease.
