Despite advances in cancer pain treatment, better understanding of pain mechanisms, and the availability of a range of novel formulations of analgesics and improved invasive techniques, in a group of our patients the result of management remains unsatisfactory. The most important factors related to pain mechanisms, properties of analgesics and individual patients that determine the unsatisfactory response to pain management will be discussed.
