The occurrence of oral cavity cancer in young adult is rare and ranges from 0.4% to 5.5%. Late diagnosis of cancer and aggressive proliferative process that makes the young patients with the oral cavity cancer begins treatment with advanced stage (usually T3 and T4). Treatment procedures involve extensive surgery with simultaneous defect reconstruction or primary chemoradiotherapy. The case of 32-year-old man with tongue cancer was presented. Primary treatment involved radiotherapy with no response. Patient’s local general condition deteriorated and was an indication to perform radical surgical treatment – salvage surgery. The authors discussed ethiopatological factors of oral cavity cancer, differential diagnosis and treatment in young adults.
