Taking psychoactive substances to initiate and/or improve the quality of sexual intercourse (chemsex) poses a real toxicological threat to individuals engaging in this practice. This is particularly true when combining several psychoactive substances, which can interact in complex ways. The author’s goal is to present a case from his own forensic practice, in which combining the psychoactive substances MDMA and sildenafil (“sexstasy”) with sexual activity ended disastrously. The primary goal of using such a mixture is to counteract the side effects of one drug with the other – to counteract the negative effects of MDMA on a man’s erection with sildenafil and to enhance sensory experiences while simultaneously reducing fatigue with MDMA. In clinical practice, appropriate patient education and awareness of the risks associated with chemsex are crucial.
