The article describes a case of a patient with a history of extensive lung cancer compressing the spinal cord in the cervical part, resulting in tetraparesis accompanied by severe occipital pain. Special attention was paid to the selection of the opioid drug, taking into account the most common and distressing adverse effect of opioid which is constipation. It is known that patients with compression of the spinal cord are particularly prone to constipation, because of bowel atony. Care for regular bowel movement is very important. After changing morphine to a combination of oxycodone/naloxone a better analgesic effect was achieved and at the same time good prevention of constipation was attained.

Conclusion: there is a group of cancer patients, whose treatment should pay special attention to the prevention of constipation. For such patients combined preparation of oxycodone/naloxone seems to be a first-line drug.
