We present a case of a 66-year-old man with squamous cell carcinoma of the tonsil, painful skeletal metastases, and coexisting chronic kidney disease after bilateral tonsillectomy and followed by radiotherapy. It is particularly difficult clinical situation due to the increased risk of pharmacological treatment with the use of non-steroidal anti-inflammatory drugs that can cause further impairment of renal function. Using twice the dose of the isotope strontium 89, in the three-month interval, several months mastering pain was achieved, without the need of daily painkillers. At the same time, control study of bone scintigraphy revealed no new evident osteoblastic lesions in skeleton. Proven high analgesic efficacy, good tolerability for the patient, and the availability of treatment under the National Health Fund should prompt physicians to consider more frequent use of this method in patients with painful bone metastases.
