Approximately 25% of inflammatory bowel disease (IBD) cases are diagnosed in childhood, with distinct characteristics compared to adult-onset disease. Transitioning from pediatric to adult care is a critical phase requiring structured planning. A literature review of PubMed, Scopus, and Web of Science articles from the past six years highlights that structured transition programs improve adherence, reduce relapse rates, and lower hospitalizations, yet many centers lack formal protocols. Key barriers include inadequate provider training, poor communication between pediatric and adult teams, and limited resources. Digital health tools and tailored education programs show promise in addressing these gaps, while cultural factors influence transition timing across regions. Standardized, evidence-based transition protocols adapted to local contexts are essential. Future research should focus on digital interventions, long-term outcomes, cost-effectiveness, and enhanced provider training. A multidisciplinary, patient-centered approach is crucial for addressing the complex medical and psychosocial needs of young adults with IBD.
