Acute cholecystolithiasis after surgical treatment of polycystic renal disease

Introduction: Autosomal dominant polycystic renal disease (ADPKD) is caused by a mutation in the PKD 1 gene. This disease leads predominantly to kidney enlargement and malformation. The first clinical symptoms appear in the third or fourth decade of life. Regardless of the symptoms at onset, the most important ADPKD complication is chronic renal insufficiency.
Aim: We indicate the possibility of complications after laparoscopic removal of kidney cysts in ADPKD treatment.
Materials and methods: We present a case of a female, 39 years old. She suffered from pain, nausea and weakness. ADPKD was diagnosed five years before and was also confirmed in the father and brother of this patient. The number of cyst was extraordinary and the largest one was 40 mm in diameter. The patient was qualified for laparoscopic cyst removal. The postoperative course was complicated by acute cholecystolithiasis.
Results: Laparoscopic cysts removal changed the anatomical relations between the operated kidney pole and liver. The position of the gall bladder changed, the gall outflow was blocked and that caused clinical manifestation of acute cholecystolithiasis.
Conclusions: Laparoscopic surgery is highly recommended and leads to regression of ADPKD symptoms. Complications after this operation may be due to changes in anatomical relations and organs’ function, not directly correlated with the surgeon’s intervention.
