Surgical treatment of atrial fibrillation in patients admitted for aortic valve replacement

Background:
In the literature there is a lack of information about the results of surgical ablation of atrial fibrillation in patients operated on for aortic valve disease. This group of patients is probably forgotten due to the majority of mitral
cases. It is not easy to find clear data about long-term results.

Aim:
Although this group is a minority among atrial fibrillation cases, we want to focus on it and to evaluate the results.

Material and methods:
In the period between 2004 and 2007, 12 patients were surgically treated for atrial fibrillation and aortic valve disease. In all cases radiofrequency ablation was performed, in 11 pts using an epicardial bipolar device. The
rhythm was evaluated in the OR, before discharge (7 days later), and during control visits 3 months and 1 year later.

Results:
There were no complications related to the procedure. In all cases sinus rhythm was restored in the OR, before discharge it was present in 58% of patients, after 3 months in 75%, and in 50% of cases one year after the operation.

Conclusions:
Surgical ablation for atrial fibrillation is a safe procedure in patients operated on for aortic valve disease. Epicardial devices allow it to be performed without a significant increase in time needed. Unfortunately 100% return of sinus rhythm in the operating room does not mean that this result will be restored in longer follow-up.
