In this paper we present the very important clinical issue of cardiac surgery in patients over 80 years with coronary artery disease. The authors discuss this problem in the context of guidelines, results from recently published studies and their long-lasting experience in the treatment of coronary artery disease. We present a report of two 89-year-old high-risk

patients who underwent coronary artery bypass grafting with cardiopulmonary bypass.
