Idiopathic basal ganglia calcification (Fahr’s syndrome) causes dementia. However, various forms of cognitive impairment and psychopathological symptoms are described in the literature and occur in practice. In the paper, a 68-year old female patient with Fahr’s syndrome is described. The process of medical and psychological diagnosis and methods of therapy are presented. The variety of clinical symptoms in Fahr’s syndrome should make us more careful in medical examination. Assessment of the neuropsychological state of these patients should be a routine examination, useful for the long-term outcome.
