Computed tomography-guided hookwire localization of solid lung tumours before videothoracoscopic surgery

Background:

Presence of a single, peripherally located change in the pulmonary parenchyma is a complex diagnostic and therapeutic problem. The introduction of videothoracoscopy allowed the diagnostic stage to be connected with the therapeutic one. However, it requires earlier localization of the change.

Aim:

To present the possibility of localization of pulmonary changes with a transthoracic hookwire under the control of CT before their removal by thoracoscopy.

Material and methods:

Twenty-three patients underwent computed tomography (CT) -guided hookwire localization of solid lung tumour with diameter range 0.5-5.5 cm (in 22 patients – single nodule). All of them underwent videothoracoscopic
resection of the localized tumour.

Results:

16 benign and 7 malignant lesions, including 2 primary and 5 metastatic, were recognized. No complications occurred during or after thoracoscopic surgery.

Conclusions:

In selected cases it may be a valuable diagnostic-therapeutic method with minimal invasiveness.
