Epidemiological Study of Suicide in Khuzestan Province, South West of Iran, during 2011 to 2014

Wstęp

Samobójstwo jest rozmyślnie podjętym czynem popełnianym przez daną osobę z pełną świadomością i oczekiwaniem osiągnięcia zamierzonego skutku [1]. Samobójstwo stanowi poważny problem z zakresu zdrowia publicznego na całym świecie. Często poprzedzone jest ono sygnałami ze strony potencjalnego samobójcy. Niektóre badania analizujące elementy zamiaru samobójczego wykazały, że ponad połowa samobójców mówiła o myślach samobójczych innym osobom. U 90% z nich rozpoznano chorobę psychiczną lub depresję [2, 3]. Inne badania wskazują jednak, że nie ma istotnej różnicy między osobami podejmującymi próby samobójcze a niepodejmującymi takich prób pod względem stopnia ciężkich psychopatologii, np. depresji, lecz istnieją istotne różnice w zakresie nasilenia poczucia beznadziei oraz wyobrażeń samobójczych. Wyniki tych badań wskazują, że na próby samobójcze mają wpływ interakcje między jednostką a czynnikiem środowiskowym [4, 5]. Według Światowej Organizacji Zdrowia w 2000 r. na świecie miało miejsce blisko 850 000 śmierci samobójczych, a próby samobójcze podejmowane były 20–40-krotnie częściej. W ciągu ostatnich 45 lat współczynnik występowania prób samobójczych wzrósł o 60%. Przewiduje się, że w 2020 r. na świecie samobójczą śmiercią zginie blisko 1,53 miliona osób [6–11]. Spodziewany jest wzrost wskaźnika samobójstw w grupie osób starszych, która stanowi najszybciej rosnącą populację świata [12]. Na świecie co 40 s ktoś traci życie, popełniając samobójstwo [13]. To jedna z 10 najczęstszych przyczyn śmierci w wielu krajach, a w grupie wiekowej 15–35 lat jedna z trzech głównych przyczyn zgonu [7, 14]. Badania wykazują również, że w USA samobójstwo jest drugą najczęstszą przyczyną śmierci w grupie wiekowej 10–24 lat, a w 2012 r. notowany był dalszy ciągły wzrost tej tendencji dla obydwu płci i różnych grup etnicznych [15]. Badania prowadzone na północy Iranu wykazują, że samobójstwo najczęściej występowało w grupie wiekowej 20–30 lat i głównie w sezonie wiosennym [16]. Inne badania pokazują, że na samobójstwo częściej decydują się kobiety, jednak wskaźnik śmierci samobójczych jest wyższy dla mężczyzn [17]. W badaniach wykazano, że ryzyko samobójstwa jest silnie powiązane z chorobą psychiczną, bezrobociem, niskim dochodem, stanem cywilnym i sytuacją rodzinną; występuje również istotna dodatnia korelacja zachowań samobójczych z niektórymi czynnikami sezonowymi, takimi jak temperatura i ekspozycja na słońce [18, 19]. Inne badania wykazują zależność liczby samobójstw od pory roku, sezonowej amplitudy temperatur, maksymalnej ilości światła dziennego i całkowitego nasłonecznienia [20]. Według raportów statystycznych Ministerstwa Zdrowia i Centrum Statystycznego Iranu najwyższe wskaźniki samobójstw notują prowincje Ilam, Lorestan, Hamadan i Kermanszah na zachodzie Iranu, a najniższe odpowiednio prowincje Sistan, Beludżystan i Kom [21]. Na podstawie wyników badania można stwierdzić, że najczęstszą etiologię samobójstwa stanowią problemy rodzinne, niski dochód i depresja [22]. Według badań prowadzonych w Iranie współczynnik występowania samobójstw w populacji ogólnej szacowano na 91,65/100 000, odpowiednio 82 dla mężczyzn i 115/100 000 dla kobiet. Na podstawie tych wyników można stwierdzić trend wzrostowy w próbach samobójczych w latach 2000–2011 [23]. W ostatnim dwudziestoleciu w dwóch miastach [24] oraz stolicach prowincji Chuzestan [25], Kerman [9] i Mazandaran [26] odnotowano znaczny wzrost liczby prób i śmierci samobójczych wśród młodzieży i młodych dorosłych. Jak doniesiono w badaniu metaanalitycznym przeprowadzonym w Iranie w 2016 r. najczęściej próby samobójcze podejmowane są w grupie wiekowej 15–24 lat, a najrzadziej przez osoby powyżej 55. roku życia [23].
Badanie przeprowadzono w celu przeanalizowania współczynników prób i zgonów samobójczych z uwzględnieniem charakterystyk demograficznych i personalnych osób popełniających samobójstwo oraz czynników powiązanych w irańskiej prowincji Chuzestan w latach 2011–2014 pod względem wpływu czynników regionalnych i kulturowych na samobójstwo oraz z potrzeby uzyskania szczegółowych informacji o tym problemie, przeciwdziałania mu i kontrolowania go. Badanie to może stanowić pierwszy krok do zdiagnozowania obecnej sytuacji w tym regionie w celu poprawy zdrowia psychicznego i w dalszej perspektywie zredukowania czynników ryzyka związanych z samobójstwem.

Materiał i metody

Opisywane badanie jest badaniem przekrojowym mającym na celu ocenę sytuacji w zakresie występowania samobójstw w prowincji Chuzestan w latach 2011–2014. Chuzestan leży w południowo-zachodnim Iranie i składa się z 23 okręgów miejskich; liczba ludności w okresie badania wynosiła 4,5 mln. Dane uzyskano z miesięcznych formularzy sprawozdawczych dotyczących samobójstw. Formularz taki został opracowany przez Ministerstwo Zdrowia i przekazany przez Wydział Zdrowia Psychicznego podległym ośrodkom zdrowia. Dane obejmowały spis wszystkich przypadków prób samobójczych i samobójstw dokonanych, do których doszło w tej prowincji w latach 2011–2014. Dane zarejestrowane dla 4 miast były niekompletne, a dane z 3 miast, które zbierał Uniwersytet Medyczny Defuel, nie były dostępne z uwagi na brak współpracy. Z tego względu miasta te nie zostały uwzględnione w badaniu. Całość badanej populacji wyniosła ok. 3,2 mln osób z 16 okręgów miejskich. Dane dotyczące prób i zgonów samobójczych z uwzględnieniem charakterystyki demograficznej, personalnej, metod i motywacji działań pochodzą z informacji zebranych w aktach oddziałów zdrowia psychicznego wydziałów zdrowia uniwersytetów medycznych w prowincji Chuzestan. Dane przeanalizowano z wykorzystaniem oprogramowania SPSS-22. Do oszacowania rocznego wskaźnika samobójstw wykorzystano roczne podstawowe dane demograficzne z Centrum Statystycznego Iranu oraz wydziału zdrowia.

Wyniki

W latach 2011–2014 w prowincji Chuzestan zanotowano łącznie 11 180 samobójstw, przy czym 60% (6728 przypadków) stanowią kobiety, zaś 40% (4452 przypadków) – mężczyźni. Stosunek kobiet do mężczyzn wśród osób podejmujących próbę samobójczą wynosił 0,65 (65 mężczyzn na każde 100 kobiet). Proporcję płci dla samobójstw dokonanych oszacowano na 1,16. Średni współczynnik prób samobójczych w omawianych 4 latach wyniósł 85/100 000, a najwyższy przypadła na 2014 r. (167/100 000). Wskaźnik samobójstw w okresie 2011–2014 wykazał istotny wzrost, z 60/100 000 w 2011 r. do 167/100 000 w 2014 r. (ryc. 1.). Średnia częstość występowania śmierci samobójczych w przeciągu omawianych 4 lat (2011–2014) wykazywała tendencję wzrostową, – współczynnik wzrósł z 2,78/100 000 w 2011 r. do 6,18/100 000 w 2014 r. (ryc. 2). Ogólnie wyniki wskazują, że samobójstwa dokonane stanowiły 3,8% całkowitej liczby samobójstw. Współczynniki samobójstw dla mężczyzn i kobiet wyniosły odpowiednio 1,5/100 000 i 9/100 000. W obrębie prowincji Chuzestan najwyższy współczynnik zanotowano dla Masdżed-e Solejman (współczynnik średnioroczny 327/100 000) na drugim miejscu znalazł się Ramhormoz (współczynnik średnioroczny 235/100 000). Najniższy współczynnik zanotowano w Hoveizeh (5/100 000). Najwyższe współczynniki samobójstw dokonanych zanotowano w Haftkel i Masdżed-e Solejman (tab. I). Najczęstszymi metodami przeprowadzenia próby samobójczej w okresie 2011–2014 były: przedawkowanie leków lub narkotyków ze współczynnikiem skumulowanym 279/100 000 (współczynnik średnioroczny 70/100 000), samobójstwo z użyciem trucizny ze współczynnikiem skumulowanym 27,6/100 000 (współczynnik średnioroczny 6,9/100 000) i powieszenie, dla którego współczynnik skumulowany wyniósł 7/100 000 (średnioroczny 1,76/100 000). Maksymalna roczna częstość sięgania po te metody przypadła na 2014 r. W przypadku samobójstw dokonanych najczęstszymi metodami były: powieszenie ze współczynnikiem skumulowanym 7/100 000 (współczynnik średnioroczny 3,24/100 000; współczynnik maksymalny w 2014 r. – 1,57/100 000), samookaleczenie ze współczynnikiem skumulowanym 2,31/100 000 (współczynnik średnioroczny 1/100 000, współczynnik maksymalny 0,58/100 000 w 2014 r.) oraz przedawkowanie ze współczynnikiem skumulowanym 1,7/100 000 (współczynnik średnioroczny 0,42/100 000; współczynnik maksymalny 0,82/100 000 w 2014 r.) (tab. II). Wyniki wskazują, że główną podaną motywacją osób, które podjęły próbę samobójczą, była niezgodność w rodzinie, ze skumulowanym współczynnikiem 72/100 000 (współczynnik średnioroczny 26/100 000, współczynnik maksymalny 18/100 000 w 2014 r.). Rodzinne nieporozumienia to również najważniejsza przyczyna samobójstw dokonanych ze skumulowanym współczynnikiem 4,27/100 000 (współczynnik średnioroczny 1,71/100 000, wartość maksymalna 1,06/100 000 w 2014 r.) (tab. I). Wyniki wskazują, że liczba samobójstw w różnych porach roku nie różni się istotnie, jednak liczba samobójstw dokonanych przypadających na jesień stanowi niemal połowę liczby samobójstw z pozostałych pór roku. Wykryto istotny związek pomiędzy występowaniem samobójstw dokonanych przez mężczyzn a porami roku w latach 2011–2014, jednak czynnik ten nie był istotny w przypadku kobiet (ryc. 3., 4.). Jeśli chodzi o metody dokonywania samobójstwa nie różniły się one w poszczególnych porach roku, a najczęstszymi metodami zarówno w próbach samobójczych, jak i w samobójstwach dokonanych były przedawkowanie leków lub narkotyków i powieszenie. Najwyższy kumulacyjny wskaźnik prób samobójczych oraz samobójstw dokonanych u obu płci zanotowano w grupie osób między 18. a 24. rokiem życia.
W tym przedziale wiekowym współczynnik skumulowany w grupie mężczyzn mających za sobą próbę samobójczą w latach 2011–2014 wyniósł 872/100 000, a współczynnik średnioroczny 218/100 000. Szczytowa wartość współczynnika samobójstw w tej konkretnej grupie wiekowej przypadła na 2012 r. i wyniosła 250/100 000. Odpowiednie wartości współczynnika skumulowanego i średniorocznego w tej grupie wiekowej dla kobiet w badanym okresie to 1167 i 292/100 000. Szczytowy wskaźnik dla kobiet z tej grupy wiekowej, wynoszący 353/100 000, przypadł na 2012 r. Współczynnik skumulowany i średnioroczny dla przypadków dokonanego samobójstwa wśród mężczyzn w wieku 18–24 lata oszacowano odpowiednio na 41 i 10,15/100 000, a szczytowy współczynnik 12/100 000 zanotowano w 2014 r. Wśród kobiet w tej grupie wiekowej współczynnik skumulowany wyniósł 36/100 000 przy współczynniku średniorocznym 9/100 000 i współczynniku maksymalnym 13/100 000 w 2013 r. (tab. III).

Omówienie

Badanie wykazało znaczny wzrost częstości podejmowania prób samobójczych pomiędzy 2011 a 2014 r., a średnioroczny współczynnik samobójstw oszacowano na 85/100 000 (60/100 000 w 2011 r. do 168/100 000 w 2014 r.). Większość prób samobójczych podejmowały kobiety, ze współczynnikiem średniorocznym 122/100 000 w okresie 2011–2014. W badaniu metaanalitycznym przeprowadzonym w Iranie szacunkowy współczynnik samobójstw wynosił 91,65, z tym że dla mężczyzn jego wartość wyniosła 82,2/100 000, a dla kobiet 115,79/100 000 [23], co jest zgodne z niniejszym badaniem. W innych badaniach prowadzonych na Tajwanie [27], w Iranie [28, 29] oraz w Chinach [30] większość prób samobójczych podejmowały kobiety, co potwierdza również prezentowane badanie. Wyniki wskazują, że 3,8% wszystkich przypadków stanowiły samobójstwa dokonane, z czego 5,1% miało miejsce wśród mężczyzn, a 2,9% wśród kobiet. Jednakże w innych badaniach uzyskano odmienne wskaźniki: od 1,1% [28] do 7,89% [27]. Większość zgonów samobójczych nastąpiło w grupie mężczyzn, co potwierdzają wcześniejsze badania obejmujące Turcję [31, 32] i Iran [28]. W Kuwejcie współczynnik śmierci samobójczych od 2002 do 2011 r. wzrósł z 3,2 do 8,1/100 000 [33, 34]. W naszym badaniu współczynnik średnioroczny samobójstw dokonanych w latach 2011–2014 został oszacowany na 3,73/100 000 przy wartościach od minimalnej do maksymalnej wynoszących 2,8 do 6,2/100 000. Dla porównania współczynnik samobójstw dokonanych w Iranie w 2010 r. oszacowano na 4,7/100 000, a najwyższy współczynnik zanotowano w prowincjach Ilam, Lorestan, Kermanszah i Hamadan – odpowiednio 26, 13, 13 i 9/100 000 [35]. W 2012 r. najwyższe współczynniki samobójstw dokonanych odnotowano w prowincjach Ilam, Lorestan i Hamadan, o wartościach odpowiednio 29, 9,3 oraz 8,6/100 000 [36]. W Turcji współczynnik śmierci samobójczych mężczyzn oszacowano na 3,95, a kobiet na 2,21/100 000 [32]. Omawiane badanie wskazuje, że najczęstszymi metodami dokonywania próby samobójczej były przedawkowanie leków lub narkotyków, zażycie trucizny i powieszenie, a wśród samobójstw dokonanych powszechniej – powieszenie, samookaleczenie i przedawkowanie. Podobne badania przeprowadzone w Chinach [30] i Szwecji [37] podają przedawkowanie leków i użycie trucizny jako najczęstsze metody wykorzystywane do popełnienia samobójstwa; inne badania wskazują, że najczęstszą metodą jest samookaleczenie [29] oraz użycie ciał stałych i płynów [27]. Badania w Turcji [31, 32], Malezji [12] i Rosji [38] wykazują, że najczęstszą metodą było powieszenie; wyniki badań są zgodne z niniejszymi wynikami. Badania w USA wskazują na postrzały, powieszenia i zażycie trucizny (przedawkowanie) jako najczęstsze metody prowadzące do samobójczych śmierci [15], a w Szwecji preferowaną metodą jest zażycie trucizny [37]. W niniejszym badaniu najważniejszą przyczyną targnięcia się na życie (próby samobójcze i samobójstwa dokonane) były nieporozumienia rodzinne; inne badania wskazują, że najczęstszymi motywami samobójstw dokonanych były choroba i problemy w rodzinie [31]. Inne badanie wymienia frustrację oraz kłopoty w pracy i problemy edukacyjne jako główne powody podejmowania prób samobójczych [30]. W omawianym artykule nie odnotowano istotnej różnicy między częstością podejmowania prób samobójczych i samobójstw dokonanych w różnych porach roku, na co wskazywały badania przeprowadzone w USA [39] i Iranie [28]. Być może jest to spowodowane tym, że w Chuzestanie jako prowincji tropikalnej zmiany pór roku nie są odczuwalne, jednak w badaniach prowadzonych w Rosji zanotowano szczytowe natężenie samobójstw w maju i wrześniu [38], a niektóre badania wykazują istotną dodatnią korelację zachowań samobójczych z niektórymi wskaźnikami sezonowymi, takimi jak temperatura i ekspozycja na słońce [18, 19] oraz korelację sezonowych prawidłowości i wahań z maksymalną ilością światła dziennego i calkowitym nasłonecznieniem [20]. W odniesieniu do poszczególnych grup wiekowych najczęściej prób samobójczych oraz samobójstw dokonanych podejmowały się osoby między 18. a 25. rokiem życia, co potwierdzają wyniki wcześniejszych badań [28, 29]. W innych krajach wyniki wskazują na inne kategorie wiekowe: w Kuwejcie wszystkie samobójstwa mieściły się w przedziale wiekowym 17–62 lat [40], w Turcji 39,2% z nich wystąpiło w grupie wiekowej 20–39 lat [31], w Chinach 50,3% osób podejmujących próbę samobójczą było w wieku 21–30 lat [30], w Malezji 16,4% zaś w wieku 15–24 lat [12]. Badania prowadzone w USA wykazują, że w grupie wiekowej 10–24 lat samobójstwo jest drugą z kolei najczęstszą przyczyną śmierci [15].

Wnioski

Dla obydwu płci w różnych grupach wiekowych najwyższy kumulacyjny wskaźnik występowania prób samobójczych oraz samobójstw dokonanych wystąpił w grupie wiekowej 18–24 lat. Najczęściej stosowaną metodą przeprowadzenia próby samobójczej było przedawkowanie leku lub narkotyku, zażycie trucizny i powieszenie, a przy samobójstwach dokonanych przeważnie powieszenie, samookaleczenie i przedawkowanie. Tendencja podejmowania prób samobójczych i samobójstw dokonanych istotnie wzrosła w okresie 2011–2014. Głównymi przyczynami prób samobójczych były kłótnie w rodzinie, wymuszone małżeństwa i rozpacz. W związku z tym zaleca się, by odpowiednie organizacje upowszechniały umiejętności związane z rozwiązywaniem problemów i zarządzaniem kryzysem oraz wdrożyły programy doradztwa życiowo-rodzinnego, udostępniły obiekty rekreacyjne służące wypełnianiu wolnego czasu młodzieży oraz, tam gdzie to konieczne, oferowały zamkniętą opiekę psychologiczną i psychiatryczną w celu zapobiegania temu społeczno-psychologicznemu zjawisku i zmniejszaniu częstości jego występowania. W formularzach Ministerstwa Zdrowia w wielu przypadkach w pozycjach motywacji i metod samobójczych wybierano kategorię „inne”, a tym samym istotne przyczyny samobójstw i metody ich popełniania pozostają nieznane. Z tego względu lista tę należy zmodyfikować tak, by oferowała bardziej szczegółowe informacje pozwalające zidentyfikować przyczynę niezbadanych samobójstw, ponieważ skuteczne planowanie odgrywa ważną rolę w ich zapobieganiu. Było to poważne ograniczenie w naszych badaniach, które miało wpływ na jego jakość i wyniki, które wpłynie również na inne badania dotyczące samobójstw prowadzone w przyszłości.

Introduction

suicide is a deliberately initiated act which and performed by the person concerned in full knowledge and expectation its completed outcome [1]. Suicide is a worldwide major public health problem. It often does not occur without warning by the person. Some studies were dissected the critical elements of suicide idea, as show more than half of people commit suicide given warning suicidal thinking which usually has shared his thoughts with others. About 90% of them diagnosed mentally ill or suffering from depression [2, 3]. However some studies show that there is no significant difference between attempters and non-attempters suicide in term of severity of acute psychopathology such as major depression but in contrast is significant in term of hopelessness and severity of suicidal ideation. Overall results of these studies indicate that attempted suicide is affected by individual and environmental factor interactions [4, 5]. According to the World Health Organization in 2000, close to 850,000 suicide deaths occurred worldwide and attempted suicide occurred 20–40 times more than suicide deaths. In addition, from 45 years ago attempted suicide rates in the world increased 60 percent. Moreover, it is predicted that in 2020, nearly 1.53 million people in the world die due to suicide [6–11]. As most rapidly growing population worldwide is comprising elderly, suicides are expected to increase in this population [12]. In the world every 40 s, someone loses its life due to suicide [13]. Suicide is one of the top ten causes of death in many countries and in the age group 15–35 years, is one of the three leading causes of death [7, 14]. Studies also show that in United States, suicide is the second leading cause of death in the age group of 10-24 years and a continued increase in both sex and different race in this group through 2012 [15]. Studies in the north of Iran show that most suicides occurred in age range of 20-30 years and mostly in spring season [16]. Other studies show that suicide is more prevalent in women than men, while the rate of suicide deaths in men is higher than women [17]. The studies show that suicide risk is strongly associates with mental illness, unemployment, low income, marital status and family history and also a significant positive relationship of suicide behavior with some seasonal indicators such as temperature and exposure to the sun [18, 19]. Other studies also show the relationship between seasonal pattern and suicide, and a positive association between the seasonal amplitude maximum daylight and total sunshine [20]. According to statistical reports by Ministry of Health and Statistical Center of Iran the highest suicide rates are occurred in Ilam, Lorestan, Hamadan and Kermanshah provinces in west of Iran and lowest in Sistan and Baluchistan and Qom provinces respectively [21]. Based on results of a study family problems, low income and depression are most common etiology of suicide [22]. According to research conducted in Iran, the incidence of suicide in the general population was estimated 91.65/100,000 which in males and females were 82 and 115/100,000 respectively. Based on these results, suicide attempts have had an increasing trend during 2000 to 2011 [23]. Based on the studies in last two decades in two cities [24] and capital of Khuzestan [25], Kerman [9] and Mazandaran [26] Provinces, suicide attempt and suicide deaths have been raising particularly among adolescents and young adults. In a meta-analysis study in Iran in 2016, it was reported that the highest rate of attempted suicide was among 15–24 year age groups and people aged over 55 years have the lowest rate [23].
Regarding the role of regional and cultural factors on suicide and the need to have detailed information about this health problem, and to prevent and control, this epidemiological study was conducted to review the suicide attempts and suicide deaths rates by considering demographic characteristics and personality of suicide attempters and associated factors in Khuzestan Province In Iran during 2011–2014. This study can help as a first step to identify the current status of the region in order to improve mental health and ultimately reduce the risk factors of suicide.

Material and methods

This study is a cross-sectional study to assess the suicide situation in Khuzestan province during 2011–2014. Khuzestan province is situated in the South West of Iran and consists of 23 Townships with an estimated population of 4.5 million in time of study. Data were extracted from a monthly suicide report form. This form is designed by the Ministry of Health and through its Mental Health Unit was instructed to undercover health centers. Data were included a census of all cases of suicide attempts and suicide deaths which had occurred in the province during 2011–2014. Recorded data of four cities were incomplete and data from the three cities which were undercover of Defuel University of Medical Sciences were not accessible due to non-cooperation. Therefore these cities were not enrolled in this study. Overall studied population were about 3.2 million from 16 townships. Data of suicide attempts and suicide deaths with regard to demographic characteristics, personality and methods and motivations of suicide were collected from recorded information in Mental Health Unit of Health Departments of Medical Universities in the Khuzestan Province .Data analyzed by using SPSS-22 software. Annual basis demographic data from Statistical Center of Iran and health department was used to estimate the annual incidence.

Results

A total number of 11180 suicides during 2011 to 2014 was recorded in Khuzestan Province, which 60% (6728 cases) were female and 40% (4452 cases) were male. The female/male sex ratio among suicide attempters was 0.65 (for every 100 women who had attempted suicide, 65 cases were men). The proportion of sex ratio in completed suicides was estimated about 1.16. The average incidence of suicide attempts over the four years was 85/100,000 and highest incidence occurred in 2014 with 167/100,000. The incidence trend of suicide from 2011 to 2014 showed a significant increases reached from 60 /100,000 in 2011 to 167/100,000 in 2014 (Fig. 1). The average incidence of suicide deaths during the four years (2011–2014) showed an increasing trend, as the incidence rate was raised from 2.78/100,000 in 2011 to 6.18/100,000 in 2014 (Fig. 2). In whole the results show that the completed suicide rate was 3.8% of all suicides. The incidence rates of completed suicides in men and women were1.5/100,000 and 9/100,000 respectively. In Khuzestan Province, Masjed Soleyman with an annual average rate of 327/100,000 was the highest and Ramhormoz with the average annual incidence of 235/100,000 ranked second. Lowest rate was occurred in Hoveizeh with 5/100,000. In cases of completed suicides the Haftkel and Masjed Soleiman cities have the highest incidence (Table 1). The most common methods of suicide attempt during the four years (2011–2014) were drug abuse with cumulative incidence of 279/100,000 (the average annual incidence of 70/100,000), suicide by poison with the cumulative incidence rate of 27.6/100,000(the average annual incidence of 6.9/100,000) and hanging with cumulative incidence 7/100,000(the average annual incidence of 1.76/100,000). The maximum annual incidence of above methods occurred in 2014. In the cases of completed suicides, the most common methods were hanging with the cumulative incidence of 7/100,000 (mean annual incidence of 3.24/100,000 and maximum incidence of 1.57/100,000 in 2014). Self-harm with cumulative incidence of 2.31/100,000 (mean annual incidence of 1/100,000 and maximum incidence of 0.58/100,000 in 2014) and drug abuse with a cumulative incidence of 1.7/100,000 (average annual incidence of 0.42/100,000 and maximum incidence of 0.82/100,000 in 2014 (Table II). The results show that main listed motivation for suicide attempters was familial discords with cumulative incidence of 72/100,000 (average annual incidence of 26/100,000 and maximum rate of 18/100,000 in 2014). In the cases of completed suicides, the most important motivation also was familial discord with the cumulative incidence of 4.27/100,000 (average annual incidence of 1.71/100,000 and peak of 1.06/100,000 in 2014) (Table I). The results show that the incidence of suicide in different seasons did not show a significant difference. However, the incidence of completed suicide in autumn was almost half of the other seasons. A significant relationship was found between incidence of completed suicide and numerous seasons among males during the years 2011–2014, but it was not significant among women (Figs. 3, 4). In case of methods of suicide there was not any different as the results show in all seasons, the most common methods of suicide attempts and completed suicide were drug abuse and hanging. Among the age groups, the highest cumulative incidence of suicide attempts and completed suicide in both sexes were in age range of 18 to 24 years.
In this age range cumulative incidence among suicide attempters in men during 2011–2014 was 872/100,000 with an average annual of 218/100,000. The Peak of incidence rate among this specific age rang occurred in year 2012 with rate of 250/100,000. While cumulative and average annual incidence in this age rang (18 to 24 years) for women in studied years was 1167 and 292/100,000 respectively. The peak incidence rate in this age range for women was 353/100,000 and occurred in year 2012. The cumulative incidence and mean annual incidence of age-specific rate of completed suicide among men in age range of 18–24 years was estimated 41 and 10.15/100,000 respectively with a incidence peak of 12/100,000 in year 2014. While among women in this age group cumulative incidence was 36/100,000 with an average annual incidence of 9/100,000 and the peak rate of 13/100,000 in year 2013 (Table III).

Discussion

In this study, the incidence of suicide attempts significantly increased from 2011 to 2014 and the average annual incidence of suicide was estimated 85/100,000 (60/100,000 in 2011 to 168/100,000 in 2014). Majority of suicide attempters were female and the average annual incidence in women was 122/100,000 during the period 2011–2014. In a meta-analysis study in Iran, incidence of suicide was estimated 91.65,which 82.2/100,000 was in men and 115.79/100,000 in women [23] that was consistent with this study. In other studies, which were carried out in Taiwan [27], Iran [28, 29] and China [30], most suicide attempters were female that confirmed our results. The results show that 3.8% of all cases were completed suicide. Which 5.1% were occurred in men and 2.9% in women. However, results of other studies show a different rates from 1.1% [28] to 7.89% [27]). Most cases of suicide deaths were among men that is consistent with previous studies, including Turkey [31, 32] and Iran [28]. In Kuwait the incidence of suicide deaths from 2002 to 2011 increased from 3.2 to 8.1/100,000 [33, 34]. In this study the average annual incidence of completed suicide during 2011-2014, was estimated 3.73/100,000 with a minimum and maximum incidence of 2.8 to 6.2/100,000. Comparing with the incidence of completed suicide in Iran in 2010 that was estimated 4.7/100,000and the highest incidences in this year were occurred in Ilam, Lorestan, Kermanshahand Hamadan with 26, 13, 13 and 9/100,000 respectively [35]. In 2012, the highest rate of completed suicide reported in Ilam, Lorestan and Hamadan with incidences of 29, 9.3 and 8.6/100,000 respectively [36]. In Turkey, incidence of suicide deaths in men was estimated 3.95 and in women 2.21/100,000 [32]. The results of this study indicate that the most common methods of suicide attempts were drug abuse, use of poison and hanging and among completed suicides, hanging, self-harm and drug abuse were more common. In similar studies in China [30] and Sweden [37], the most common methods used for suicide were listed as drug abuse and the use of poison and other studies show that the most common methods of suicide attempts were self-harm [29] and the use of solids and liquids [27], studies in Turkey [31, 32], Malaysia [12], Russia [38] show that the most common method of suicide deaths was noted as hanging in which the results were consistent with our results. Studies in United States indicate that the most common methods of suicide deaths were gunshots, hanging and the use of poison (drug overdose) [15] and in Sweden the poisons were used rather than other suicide methods [37]. In this study, Family discord was listed as the most important motivation for suicide (suicide attempters and completed suicide), while the results of other studies show that disease and family problems was listed as the most common motivation among completed suicides [31]. In another study Frustration, work and educational problems were mentioned as the main causes of suicide attempts [30]. In this study, there was no significant difference between seasonal incidence of suicide attempts and completed suicides, as other studies in the United States [39] and Iran [28] confirmed these results This may be because of uniform geographical season of Khuzestan as a tropical Province, however a study in Russia show a peak of suicide in May and September [38], and some studies show a significant positive relationship of suicide behavior with some seasonal indicators such as temperature and exposure to the sun [18, 19] and seasonal pattern and amplitude and maximum daylight and total sunshine [20]. Among age groups, the highest incidence of suicide attempts and completed suicides were in age range of 18–25 years which confirmed by previous studies [28, 29]. In other countries the results are demonstrated with different age categories in Kuwait, all suicides were between age range 17–62 years [40], in Turkey 39.2% were in age range of 20–39 years [31], while in China, 50.3% of suicide attempters were between 21–30 years [30] and in Malaysia 16.4% was between 15–24 years [12]. Studies in United States show that the among 10–24 year-old age groups, suicide is the second leading cause of death [15].

Conclusion

Among the age groups of both sexes, the highest cumulative incidence of suicide attempts and completed suicide were occurred in age range of 18 to 24 years. The most common methods of suicide attempts were drug abuse, use of poison and hanging and completed suicides mostly seen due to hanging, self-harm and drug abuse. The incidence trend of suicide attempts and completed suicide significantly increased from 2011 to 2014. The main reasons and motives of suicide attempts were family discords, forced marriages and despair, respectively. Based on these results it is suggested that concerned organizations provide problem solving skills, crisis management and establish the consultative programs related to issues of life and marriage, recreate facilities on order to fill the young people’s leisure time and where are necessary psychological and psychiatric inpatient services to prevent and reduce the incidence of this social – psychological health matter. The motivations and methods of suicide in many cases in used check list of Health Ministry are listed in category “other”. So considerable causes and methods of suicides are unknown and unstated.Therefore the list should be modified to provide more detailed information for discovery impetus behind of these all unknown suicides for effective planning comes in suicide prevention. This was serious limitation of our research which affects the quality and outcome of the study. This also will influence other studies related to suicide in future.
