Current opinions about radicular pathology and origin of pain in degenerative disc disease are presented. Increasingly, many authors point to the inflammatory process in pathophysiology of morphological and functional nerve root changes and radicular pain origin. Now it is clear that the nucleus pulposus has antigenic properties and may be a source of many inflammatory mediators, particularly TNFα. Observations from the first clinical investigations including biological therapy of radicular pain and root dysfunction in disc disease are favourable, but too limited. Therefore many more clinical multicentre, randomized trials are needed, to answer in depth the question about the effectiveness and usefulness of biological drugs in routine clinical therapy of radicular pain syndrome
