Introduction:
Despite the endemicity of cutaneous leishmaniasis, mucosal involvement remains rarely observed. Lip involvement causes mutilations of varying severity.

Objective:
This article aims to describe a novel technical repair of a partial thickness vermillion defect.

Case report:
A 27-year-old female patient asked us for cosmetic repair of visible leishmaniasis sequelae on the upper lip causing her discomfort due to difficult to hide defect of her red lip. Scar excision resulted in red lip defect repaired by two juxtaposed flaps, inspired by the Goldstein flap, allowing for successful results.

Conclusions:
Our proposal of a flap based on the superficial layer of the vascular network, inspired by the Goldstein flap, is a simple and reliable alternative to the repair of red lip defects providing optimal aesthetic and functional results. Flap harvest can be single or double depending on the area to be covered.
