Introduction:
Lichen planopilaris is a chronic, autoimmune disease that leads to permanent hair loss. Although it has characteristics of scarring alopecia, which constitutes a contraindication to hair transplantation, there are new reports of successful procedures being performed in lichen planopilaris.

Objective:
To present a case showing the potential use of hair transplantation in the treatment of hair loss in lichen planopilaris.

Case report:
A 36-year-old male patient with a hair loss in the occipital part of the scalp as a result of lichen planopilaris, diagnosed 6 years earlier, was studied. After assessment of the hair loss area, the patient was qualified to undergo a hair transplantation procedure. The effect of the procedure was finally evaluated after a year and found to be satisfactory.

Conclusions:
Hair transplantation procedures seem to have a great potential in the treatment of hair loss in lichen planopilaris. However, a very careful assessment and qualification of patients have to be performed.
