Bioceramic putty versus calcium hydroxide for direct pulp capping in immature permanent molars: a 12-month randomized controlled trial

Introduction:
Vital pulp therapy as direct pulp capping is crucial for preserving pulp vitality in immature permanent molars to ensure natural root development.

Objectives:
To compare the effectiveness of bioceramic putty (BC) and calcium hydroxide (CH) as direct pulp capping agents in immature permanent first molars (PFM) with exposed pulp tissue during a randomized controlled trial over a 12-month follow-up time.

Material and methods:
A total of 50 participants with deep caries affecting their immature PFM were recruited. After caries removal and occurrence of pinpoint pulp exposure, 36 participants were enrolled in the current study. Participants were randomized and divided into two groups (n = 18 each group). In group 1 (control group), patients were treated direct with pulp capping (DPC) using CH, while in group 2 (tested group), patients were treated with DPC using BC material. For the assessment of success or failure, participants were examined clinically and radiographically at 3-, 6-, and 12-month post-treatment visits.

Results:
There were non-significant variations in the success rates amongst both the groups at any of the assessed time intervals. However, there was a non-significant (p = 0.23) decrease in the success rate of CH over time, with a higher number of failures observed at 12 months, compared with 3 and 6 months. Moreover, the BC group consistently had only 1 unsuccessful patient at each follow-up visit.

Conclusions:
Bioceramic putty and CH are similarly effective for direct pulp capping of permanent teeth with pinpoint pulp exposure in the occlusal cavity.
