A twenty-seven year-old woman with 5 years’ history of rheumatoid arthritis was admitted to the hospital because of chronic cough and dyspnoea on exertion. Pulmonary function tests revealed a severe obstruction with dynamic lung hyperinflation, a decrease in the lung diffusion capacity and negative bronchodilation test. High resolution computed tomography revealed a few fibrotic changes in the right lung only, without symptoms of air trapping. The diagnosis of bronchiolitis obliterans was established. The patient was treated with prednisone 1 mg/kg/day and cyclophosphamide 1 mg/kg/day. There was no improvement. The patient was qualified for lung transplantation.
