Neoplastic fever

Fever is a paraneoplastic symptom, that can be used to predict the diagnosis of neoplasm as well as indicate its recurrence or progression. Prolonged fever is an unfavourable symptom for patients in advanced stage of incurable disease, which can lead to dehydration, progression of cachexia, and distress. Neoplastic fever represents a serious diagnostic dilemma. There are no specific features allowing a proper diagnosis based only on the clinical picture. The diagnosis of neoplastic fever requires prior exclusion of other causes of fever, especially infections, being the most common cause of elevated temperature in cancer patients. The naproxen test should be considered in the absence of contraindications for nonsteroidal anti-inflammatory drugs because of its high positive predictive value for the diagnosis of neoplastic fever in cancer patients. Early diagnosis of neoplastic fever can help avoid unnecessary antibiotic therapy and its complications.
