The ileal pouch-anal anastomosis is a well-established surgical treatment for various colorectal diseases in children, such as ulcerative colitis, familial adenomatous polyposis, and Hirschsprung disease. Despite its extensive use in adults, pediatric cases present unique challenges that require specialized consideration. This review provides a comprehensive analysis of the indications, surgical approaches, outcomes, and long-term management of intestinal pouches in children. We emphasize the importance of multidisciplinary care, individualized surgical planning, and long-term follow-up to achieve optimal outcomes for pediatric patients.
