Management of low back pain – treatment strategies

Low back pain is becoming one of the most serious problems in present-day medicine. This paper briefly reviews the diagnostic, clinical and therapeutic problems of low back pain. The effectiveness of medication (pharmacological and non-pharmacological) for acute and chronic low back pain is discussed. The recent data suggest that therapies with good evidence of moderate efficacy for chronic or subacute low back pain are cognitive-behavioural therapy, exercise, and interdisciplinary rehabilitation. The only therapy with good evidence of efficacy for acute low back pain is superficial heat. In conclusion, therapeutic management should be inter-disciplinary and focused on cooperation between different specialists. On the other hand, patient preferences, cost, availability and clinical features are important for making an appropriate decision concerning treatment strategy.
