Urinary tract infections (UTIs) are the most common infectious diseases. The recommendations regarding the diagnosis, treatment and prophylaxis of UTI in children have been changed in the last years. The most important of them were presented below. Early diagnosis and appropriate treatment, initially empirical depending on the clinical condition of the child and in accordance with local antimicrobial sensitivity patterns, and then modified according to the results of antibiogram, play an important role. Diagnosis is made, apart from clinical assessment of the child, on the basis of pyuria and urine culture. Asymptomatic bacteriuria does not require treatment. Aggressive imaging of all children after the first episode of febrile UTI is not recommended. The urinary system ultrasound is a basic imaging method. Antimicrobial prophylaxis is not recommended. Risk factors for UTI should be identified and eliminated in any case of child with UTI.
